Description Community Health Centers of America is a premier multi-specialty group focused on improving the health of the community through excellence in clinical care. We are looking for a Psychiatric Nurse Practitioner to work with us part-time (2-3 days/week) at clinics in the greater Sacramento area with both in-person visits and telehealth appointments. POSITION SUMMARY: Diagnoses and treats patients struggling with mental health and mental illness including but not limited to Issues such as anxiety, depression, PTSD, mood disorders, and substance abuse issues, postpartum depression among other mental health and illness issues. REQUIRED COMPETENCIES: Provide direct psychiatric medical services to include examination, diagnosis, treatment, referral and follow-up of patients in clinic setting. Ability to interact effectively and in a professional, supportive manner with persons of all backgrounds Excellent verbal and written communication skills. ESSENTIAL DUTIES: Provide a safe, educational experience for patients while upholding the standard of care per the Medical & Nursing Boards of California. Educate patients regarding their diagnoses, treatment and prognosis. Provide all services in accordance with accepted medical standards, organizational policies, contractual requirements, and in a manner that is responsive to the needs of all patients. Maintain complete, accurate and legible medical records according to established protocols Participate in quality assurance, peer review and utilization review activities as assigned. PRIMARY ACCOUNTABILITIES: Obtains, analyzes and interprets patients' histories, symptoms, physical findings, or diagnostic information to develop appropriate diagnoses. Applies evidence-based practices and best practices critical time intervention and trauma informed care. Performs and gives psychotherapy for the treatment of conditions. Performs professional work that requires specialized to general knowledge of fields of advanced practice nursing, and a solid understanding, use, and application of concepts, theories, principles, practices, terminology, and applied bases of area of specialization, specifically psychiatry mental health, and addictions. Prescribes medications. Develops a nursing assessment tool to reflect the holistic needs of each client with consideration of patient’s psychological, physiological, social, ethnic, cultural, educational and spiritual strengths and needs. Oversees the direct implementation of nursing care to ensure that it is carried out in accordance with the plan of nursing intervention and overall treatment/service plan and to ascertain if the needs of the client require modification of the plan. Maintains complete and detailed records of patients' health care plans and prognoses. Detects and responds to adverse drug reactions. Keeps abreast of developments in nursing. Schedules follow-up visits to monitor patients or evaluate health or illness care. Maintains current knowledge of state legal regulations for nurse practitioner practice. Keeps abreast of regulatory processes and payer systems. Obtains health information from patient. Practices safe and healthy control methods while working with patients. Educates patients and families concerning medical treatments, medications, etc. Participates in team meetings and provides nursing input on the development and implementation of treatment/service plans. Interprets test results to diagnosis any related health issues. Counsel patients about compliance with medication. Coordinates activities of patient care. Documents patients’ progress with medication or treatment. OTHER RESPONSIBILITIES: Follow Community Health Centers of America’s rules, policies, and procedures while on assignment. Follow OSHA safety rules and procedures while on assignment. Respect cultural and religious practices of patients. Uphold HIPAA regulations. Punctual and dependent for assigned/confirmed shifts.
