Description
Description The Psychiatric Nurse Practitioner will address the behavioral health needs of the clients by conducting psychiatric evaluations, prescribing and managing psychotropic medications. The Psychiatric Nurse Practitioner will coordinate with other behavior health and primary medical providers and support care management within Haymarket Center Patient Centered integrated model. The candidate will be certified (waivered) in the treatment of drug addiction and will assess and assist with staff development related to behavioral health and addiction treatment. The primary function of the Psychiatric Nurse Practitioner is to diagnose, prescribe medication, monitor side effects and participate in the treatment process. Uses current evidence-based practice, research and clinical knowledge to improve patient outcomes and reduce complications. Conducts mental health histories and assessments. Formulates behavioral health treatment plan and manages acute and chronic mental health needs within Primary Care Mental Health model of care. Provide treatment to clients with drug/opioid addiction. Provide assessment and medication consultation for primary care and behavioral health providers for patients with complex behavioral health, developmental, or substance disorders. Orders laboratory and other diagnostic tests and evaluates data obtained. Support the stabilization of patients discharged from mental health hospitalization back into primary care for follow-up and ongoing management. Consult with medical providers on alternative or appropriate therapeutic drug regimens and other treatment strategies. Attend required clinical meetings and conduct training courses based on staff needs. Document in accordance with all regulatory requirements made by licensing and third-party agencies. Completes other duties as may be assigned by the immediate supervisor that are consistent with the responsibilities of the position. Requirements • Master’s degree in Nursing. • Graduation from accredited Master’s level Nurse Practitioner program with a specialty in psychiatry. • Licensed to practice as nurse practitioner in Illinois • A collaborative agreement with a qualified physician. • Board certified or eligible as a Mental Health Nurse Practitioner. • Holds current or eligible for Data 2000 waiver. • A minimum of 3 years’ work Experience in Behavioral Health preferred. • 2 years of experience in outpatient setting with knowledge of FQHC program preferred. • Computer Skills. Experience with Excel and Word. • Proficiency in use of electronic medical records. • Community based experience.
