Responsibilities of the Psychiatric Nurse Practitioner • Provide psychiatric assessments, medication management, and ongoing clinical oversight to adults enrolled in the ACT program. • Deliver community-based care, meeting clients in their homes or other community settings as needed. • Collaborate with a multidisciplinary team including psychiatry, social work, nursing, vocational specialists, and peer support providers. • Participate in daily team meetings and treatment-planning sessions to ensure coordinated, person-centered care. • Maintain accurate and timely clinical documentation in accordance with regulatory standards. • Contribute to crisis intervention, risk assessment, and development of safety plans when necessary. • Support recovery-oriented, culturally responsive, and trauma-informed approaches to care. Requirements of the Psychiatric Nurse Practitioner • Active New York State Nurse Practitioner License (Psychiatric–Mental Health Nurse Practitioner certification required). • Master’s or Doctoral degree from an accredited Nurse Practitioner program. • DEA registration or eligibility to obtain. • Experience working with individuals with serious mental illness; ACT Team or community-based psychiatric experience preferred. • Valid driver’s license and ability to travel within Westchester County for community visits. • Strong communication, organizational, and interdisciplinary-collaboration skills.
