JOB DESCRIPTION: HOPE FOUND INC. IS LOOKING FOR QIDP WHO IS EITHER A LICENSED PSYCHOLOGIST, OR A CLINICAL SOCOIAL WORKER OR A LICENSED PROFESSIONAL COUNSELOR.

• QIDP develops, coordinates and monitors all aspects of treatment, be it supported living, host home services, in-home support services or other residential services.
• Develop and maintain residential behavior support plans.
• Responsible for assisting individual’s living in their preferred lifestyle by developing a person centered support plan, consistently reviewing to ensure plan is being carried out and regularly reviewing options with people to see if their preferences or needs have changed.
• Monitor progress of program plans. Respond to deficiencies in services provided.
• Conduct Incident Investigations as it relates to individuals on their caseload.
• Provide training to Direct Care staff and oversight.
• Reviews daily staff documentation, communication book, log book and respond to deficiencies and items as needed.
• Collaborate with Program Director, Nursing & House Manager, & HR Department to ensure staff coverage and transportation for medical appointments and ensure that appointments are met.
• Coordinate training with Nurse, PT, OT, Speech, Behavior Specialist, and any other Clinical Services to ensure staff receiving trainings.

GENERAL REQUIREMENTS:

• Experience working with people with developmental disabilities.
• Possession of a valid driver’s license, good driving record, and own transportation.
• CPR & First Aid certified.
• At least two years of supervisory or management experience.
• Bachelor’s Degree in Human Services or related field

  Pay: $58,552.64 - $70,000.00 per year
