Responsibilities

ABOUT THE ROLE

As a Support Worker in our team, you will be supported to work beside individuals to build their capacity with daily living skills, assisting with social and recreational activities, implementing and monitoring practices that will promote a healthier lifestyle and empower participants to help them achieve their goals. 
The role will involve working with a participant including recoveries and complex needs. This participant may need assistance with medication prompts and may show complex behaviors. Experience and professionalism in mental health are highly Regarded.
You will be required to work in the participant’s home, or in the community wherever the need is required

KEY RESPONSIBILITIES

· Provide person-centered support to clients with complex and/or challenging needs
· Provide transportation support to take participants to appointments, social and community access so a reliable vehicle will be required. 
· Build rapport and collaborate effectively with the participant, their families, friends and care team to achieve positive outcomes
· Be a mentor and encourage individuals to learn and undertake daily living tasks
· Provide personal care and assistance with personal and domestic activities
· Monitoring the well-being of clients and reporting on their progress
