Planned Parenthood Northern California (PPNorCal) is seeking a Quality Assurance Clinician in Concord, CA. This role is part-time, 24 hours per week with a hybrid schedule. This position primarily collaborates with the Clinical Quality Assurance team and the Sr. Director of Clinical Quality Assurance to conduct medical record audits and determine corrective actions, review and evaluate medical incident reports, and assess the organization’s medical systems and processes to ensure compliance and continuous improvement (e.g., participate in the PPNorCal routine site visit audit). Under the guidance of the Senior Director of Clinical Quality Assurance, Director of Clinician Training and Clinical Development, and the Medical Director, and in partnership with the Director of Clinical Practice, the Quality Assurance Clinician provides essential support and maintenance of Epic templates, workflows, and helpful documentation tools such as job aids and training resources related to the OCHIN Epic system. The Quality Assurance (QA) Clinician dedicates 24 hours of weekly work to project based quality assurance and Epic documentation administrative activities, often working independently and with remotely.     ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. * Complies with PPNorCal Medical Standards and Guidelines to ensure safe, evidence-based clinical practices. * Delivers all responsibilities with professionalism and a patient-centered approach, in alignment with PPFA’s Manual of Medical Standards and Guidelines (MS&Gs), PPNorCal policies and procedures, and applicable state and federal regulations. * Contributes to the delivery of high-quality health care by actively engaging in quality assurance and medical risk management initiatives, ensuring compliance with PPFA, Title X, state licensing requirements, CLIA, and OSHA standards. * Demonstrates advanced proficiency in Microsoft Excel, Word, PowerPoint, and Epic to support clinical audit performance, medical incident tracking, report writing, and professional presentations. * Fosters a culture where clinical quality and safety are shared responsibilities, emphasizing system-focused performance improvement and accountability without blame. * Collaborates with the Sr. Director of Clinical Quality Assurance to oversee the Clinical Quality Improvement Program, ensuring alignment with organizational goals. * Under the direction of the Sr. Director of Clinical Quality Assurance, executes the annual audit schedule in accordance with PPFA and Title X requirements. 1. Assists with data collection and analysis to support audit findings. 2. Identifies areas of clinical risk and recommends mitigation strategies. 3. Supports health centers in developing and implementing corrective action plans, ensuring measurable improvements. 4. Presents audit results, corrective actions, and progress updates to the Clinical Quality Assurance Team. 5. Collaborates with program directors and managers to enhance the quality and effectiveness of clinical programs. * Participates in Clinical Leadership, Clinical Quality Assurance, and Epic Documentation Workgroup meetings to discuss audit findings, corrective actions and uses data to inform strategic decisions and continuous quality improvement (CQI) initiatives. * Collaborates with the Clinical Leadership Team to review and disseminate updates to medical standards and guidelines. 1. Serves as a clinical resource for affiliate clinicians, registered nurses, center directors, and staff regarding medical protocols, policies, and procedures. 2. Contributes to the development and revision of medical forms and patient education materials. * Participates in the annual health center site visit process to support quality and compliance. * Presents clinical cases and findings to the Peer Review Committee to support continuous learning and quality improvement. * Epic Certification, Clinical Build, and Documentation Compliance  1. Obtains and maintains Epic Clinical Content Builder certification 2. Manages and maintains clinical documentation within the OCHIN Epic DEV environment, including Notes, SmartSets, SmartPhrases, SmartTexts, and Patient Education Sheets, to ensure alignment with current PPFA Medical Standards & Guidelines. 3. Collaborates with program directors and the training team to optimize documentation workflows. 4. Submits and tracks Epic clinical content management tickets through OCHIN Jira, providing regular status updates to internal stakeholders. 5. Reviews and assesses EpicCare Ambulatory change requests, maintains a centralized repository of clinical build requests, delivers timely feedback to requesters, ensures integration into the EHR, and coordinates updates with training initiatives. 6. Revises Epic job aids and communicates documentation updates via email and Hot Flashes. 7. Participates in the ADM-123 process for new clinical initiatives (e.g., medications, labs, and procedures) by submitting Jira tickets for code investigation and collaborating with Clinical Operations, E.H.R. Support, and Revenue Cycle teams for testing as needed. * Provides support for grants and special projects as assigned. * Engages in agency-wide task forces to advance organizational initiatives. * Role-models and champions leadership in the implementation of PPNorCal’s Diversity, Equity, and Inclusion (DEI) plan.   ESSENTIAL DUTIES AND RESPONSIBILITIES OF ADVANCED PRACTICE CLINICIAN include the following. Other duties may be assigned:    * Works in the health center providing comprehensive clinical sexual and reproductive health care services to all patients in accordance with evidence-based practice protocols.  * Utilizes knowledge of and experience in patient care to:   * Obtains a basic health history and comprehensive gynecological (GYN) history.  * Performs a general physical exam, breast exam, GYN exam, and male genitourinary exam    * Recognizes common breast and GYN deviations from normal and provide management or referral as appropriate   * Recognizes common non-GYN deviations from normal and provides management or referral as appropriate   * Provides preventive care, including screening for breast and cervical cancers    * Obtains and interpret laboratory tests and results related to GYN visits   * Performs and interpret transvaginal and abdominal ultrasounds   * Screens for contraindications and special conditions to contraceptive methods    * Provide appropriate contraception to patients, including intrauterine contraceptives  (IUDs) and implants   1. Tests and treats STDs and other infectious conditions 2. Provides PrEP and nPEP to prevent HIV infection  3. Provides care across the reproductive health lifespan, including basic infertility services and menopause management  4. Provides medication abortion, early pregnancy complication management, and other special services as appropriate by licensure  5. Assesses and manage complications from surgical abortion, medication abortion, and vasectomy  6. Provides care for patients of all gender identities, including gender-affirming care 7. Exercises a commitment diversity, equity, and inclusion in all interactions with patients and staff  8. Constructs an accurate assessment and plan based on history, exam, and lab findings  9. Provides patients with general reproductive health maintenance, education, and counseling  10. Ensures complete and accurate medical documentation  11. Acts as a medical resource for health center staff  12. Participates in health center efforts to achieve established goals for productivity, with the goal of 3.0 visits per hour  13. Bills and codes patient visits appropriately  14.
