Quality Improvement Specialist Muskegon Family Care 2201 S. Getty St. Muskegon, MI 49444 FULL-TIME M-F 8 AM-5 PM Muskegon Family Care is looking for a Full- Time Quality Improvement Specialist to join our Supportive & Collaborative Team. Position Overview Under the guidance of the Quality Manager, the Quality Improvement Specialist contacts patients to schedule due/overdue preventative and chronic condition services. Essential Duties and Responsibilities * Review charts for missed services and schedule patients for appointments. * Assembles gift bags and fit kits * Pull UDS Measure reports and contact patients to schedule appointments utilizing Azara registry. * Contacts patients to attend smoking cessation classes. * Fit Kit pick up- contact patients to complete fit kit and drive to collect completed fit kits. * Works missed services reports received from QI Analyst. * Answers telephone and reply to voicemail messages. * Enter data into patient electronic medical records when necessary. * Orders due to preventive services when necessary. * Maintains a courteous, helpful and conscientious attitude in dealing with co-workers and patients and their families, always ensuring professionalism and confidentiality. * Understands all aspects of Healthcare Effectiveness Data and Information Set (HEDIS) scores and reporting. * Understand and implement Patient Centered Medical Home initiatives to increase quality of care and provider reimbursement from BCBSM. * Understands and implements healthy People 2020 goals and initiatives to improve measures. * Regular and prompt attendance at work. * All other duties assigned. Education and Licensure * High school diploma or GED (Required). * Registered or Certified Medical Assistant preferred. * CPR certification required within 6 months of employment. * Three to six months’ related experience and/or training preferred. * Computer and Medical Terminology experience preferred. Responsibilities
