Quality Improvement Specialist Muskegon Family Care 2201 S. Getty St.  Muskegon, MI 49444   FULL-TIME M-F 8 AM-5 PM   Muskegon Family Care is looking for a Full- Time Quality Improvement Specialist to join our Supportive & Collaborative Team. Information/eligibility is available from the U.S. Department of Education.   Position Overview Under the guidance of the Quality Manager, the Quality Improvement Specialist contacts patients to schedule due/overdue preventative and chronic condition services.     Essential Duties and Responsibilities   * Review charts for missed services and schedule patients for appointments. * Assembles gift bags and fit kits * Pull UDS Measure reports and contact patients to schedule appointments utilizing Azara registry. * Contacts patients to attend smoking cessation classes. * Fit Kit pick up- contact patients to complete fit kit and drive to collect completed fit kits. * Works missed services reports received from QI Analyst. * Answers telephone and reply to voicemail messages.  * Enter data into patient electronic medical records when necessary. * Orders due to preventive services when necessary. * Maintains a courteous, helpful and conscientious attitude in dealing with co-workers and patients and their families, always ensuring professionalism and confidentiality. * Understands all aspects of Healthcare Effectiveness Data and Information Set (HEDIS) scores and reporting. * Understand and implement Patient Centered Medical Home initiatives to increase quality of care and provider reimbursement from BCBSM. * Understands and implements healthy People 2020 goals and initiatives to improve measures. * Regular and prompt attendance at work. * All other duties assigned. Qualifications To perform this job successfully, an individual must be able to perform each essential function satisfactorily. Knowledge of medical terminology and medical abbreviation.   Education and Licensure * High school diploma or GED (Required). *  Registered or Certified Medical Assistant preferred. * CPR certification required within 6 months of employment. * Three to six months’ related experience and/or training preferred. * Computer and Medical Terminology experience preferred. Responsibilities
