Benefits: 401(k) Health insurance Paid time off Vision insurance Primary Care | On-Site | Full-Time Salary: $20.00 – $28.00 per hour, based on licensure, experience, and qualifications Job Description We are seeking a detail-oriented Quality Initiatives Specialist to support quality improvement, population health, and value-based care initiatives within our primary care clinic. This on-site role focuses on care gap closure, preventive care coordination, patient outreach, and collaboration with payers and clinical staff to improve patient outcomes and quality performance. This position is ideal for a Medical Assistant (MA) or Licensed Practical Nurse (LPN) with strong organizational skills and an interest in quality programs and proactive patient care. Key Responsibilities Communicate with insurance payers regarding quality programs and care gap initiatives Review care gap lists, population health reports, and quality dashboards Perform patient outreach to schedule office visits, annual wellness visits, preventive screenings, labs, imaging, and diagnostic tests Collaborate with providers and clinical staff to close care gaps and improve quality outcomes Support accurate documentation of quality measures within the electronic health record (EHR) Track and maintain quality performance data and reports Educate patients on preventive care, chronic disease management, and recommended screenings Assist with payer audits, chart reviews, and quality data submissions Maintain HIPAA compliance and patient confidentiality at all times Required Qualifications Certified Medical Assistant (MA) or Licensed Practical Nurse (LPN) – required Experience in a primary care, family medicine, or outpatient clinic preferred Experience with quality measures, care gap closure, HEDIS, Medicare Advantage, or population health strongly preferred Strong communication, organizational, and follow-up skills Comfortable with patient outreach and care coordination Proficient with EHR systems and basic computer applications Ability to manage multiple priorities with attention to detail Work Schedule & Location On-site position in a primary care clinic Monday–Friday, daytime hours No evenings or weekends Compensation & Benefits Hourly pay: $20.00 – $28.00, based on experience and licensure Benefits package may include medical insurance, paid time off, and holidays Hiring Timeline Candidate review and interviews are expected to begin the week of January 19, 2026.
