Minimum qualifications: Bachelor’s degree in health or quality assurance field, plus two years related experience. Knowledge of Joint Commission expectations and processes, NCQA Patient Centered Medical Home, and Community Health Centers preferred. Excellent organizational, analytical, and problem-solving skills. Proficient verbal and written communication skills. Convenient if bilingual in English/Spanish. Ability to multi-tasks and manage disparate assignments well. Highly skilled to speak clearly and concisely as well as read, understand, and follow oral and written instruction. Day to Day (this list may not all include all the duties assigned): • Responsible for assisting the organization with implementation of processes specific to accreditation, quality improvement and assurance, patient safety, patient relations. • Ensure alignment with organizational goals and regulatory requirements. • Facilitate the scheduling of patients with identified care gaps and the reduction of barriers to care. • Provide education to patients with identified care gaps regarding the importance of addressing these gaps for health and well-being. • Performs necessary data entry to assist in quality assurance activities. • Research information related to quality improvement and quality & accreditation as needed/directed. • Assist in coordinating meetings, activities, and events as needed. • Assist Chief Quality and Compliance Officer as needed and/or directed. • Assist administrative staff as needed and/or directed. • Represent YNHS in local meetings as requested. • Maintain organized, clean, efficient and confidential work area. • Perform related work and other tasks as requested.
Responsibilities
The Quality & Population Health Specialist is responsible for assisting with the implementation of processes related to accreditation, quality improvement, patient safety, and patient relations. This role also involves facilitating patient scheduling and providing education to patients regarding care gaps.
