Description:

The Brent Rapid Response team is an unique service that aims at providing a high quality clinical service in the patient’s own home and preventing unwarranted admissions to hospitals. The service benefits from a comprehensive multi-disciplinary presence and works under the medical model. An opportunity has risen in the team for the position of a Band 5 Nurse which will be seen as an exciting opportunity for Junior Nurses who are looking for a community position.
To provide skilled nursing care working under the supervision of Senior Rapid Response Nurses to patients in a variety of community settings and hospital A&E. The post holder will undertake Rapid Response assessment duties involving delivery of nursing services and holistic assessments of patient’s health needs using a person-centered approach. This includes providing quality and seamless nursing care to housebound patients and being proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care institutions. In addition, the post holder will be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery.
