Job Description Candidates will work at the Mecklenburg County Sherriff's Office (MCSO) to outreach to the incarcerated popoulation. Identifies eligible candidates, facilitate program orientations, complete evidence-based risk/needs assessment Maintains a caseload, builds transition plans for clients to include individual case planning, composes and collets required documents Assists participants with completing various forms and obtaining documents for transition, training, education, and employment Reviews documents for accuracy, completeness, and compliance Compiles data for reports as needed Assists participants in gaining and retaining employment and provides referrals as necessary Networks with community providers, employers, and partners to establish vital contacts for case planning, training, education, and employment Tracks and evaluates participant’s program compliance and non-compliance by working with agency divisions, partners, vendors, and re-entry team Resolves participant requests regarding payment issues, free-time, special purpose passes, and grievances Maintains and adheres to site policies, procedures, and regulations and ensures confidentiality of all data/information Other duties as assigned Qualifications High School Diploma or equivalent with two years of related experience preferred Excellent computer skills and knowledge of Microsoft Office, Word, Excel, Power Point Skills in communicating with diverse populations Experience performing case management and transition plans preferred Ability to work independently and have well developed interpersonal skills Experience in criminal justice or related human service field preferred Ability to travel as needed at least 25% Additional Information All your information will be kept confidential according to EEO guidelines.
