Reablement Coordinator - Ref: SS05425 G4: £26,403 - £28,142 per annum. Are you a confident and effective communicator who enjoys working with others? Do you have good IT skills, being able to use different software programmes? Do you thrive in a busy environment and have the skills to work on your own initiative as well as being a team player? As a Community Reablement Co-ordinator you will play a vital role through brokering services, communicating with external providers, colleagues and residents to deliver and maintain a high standard in service provision. Utilising your IT skills will ensure information is accurately recorded on the appropriate systems relating to purchasing of services.
