Reablement Liaison Officer – 2 posts | 10287 | permanent contracts | 22 hours or 22.75 per week | £26,824 to £28,142 per annum pro rata | Grade E | Dereham/hybrid working Do you have excellent customer services and IT skills with a passion for Social Care? If so, we’d love to hear from you. We have an opportunity to join our South Norfolk team as a Reablement Liaison Officer. In this role, you will be the first point of contact and an information service for GPs, community health services, social services, service users and their families.  You will also: * Take referrals for Norfolk First Support.  * Take referrals into our service by phone and Liquid Logic (our social care system). * Set up visits to service users for support workers using an electronic scheduling software (full training will be given). * Take queries from the support workers while they are working in the community.   Norfolk First Support delivers services to adults across Norfolk in their own homes for up to 6 weeks, assisting them to regain skills they may have lost following an illness or hospital admission, enabling them to regain their independence as much as possible while also reviewing any ongoing support needed. You need excellent IT skills and be able to work to tight deadlines, often making decisions independently whilst delivering excellent customer service. You will also be able to assess risk and take appropriate action, working with our multidisciplinary team to support people to return to their homes.  The role is hybrid, working both from home and a locality office. Our rota is on a two-week or four-week rota basis, which includes working morning and evening shifts, alternative weekends and bank holidays, but this is planned in advance to help us deliver our service whilst enabling you a healthy work-life balance. Weekend hours are paid at a higher rate of time and a third.
