Responsibilities

THE ROLE:

In this rewarding role, you will champion person-centred care, ensuring that client goals are continuously reviewed and achieved throughout their care journey. You will assess clients’ needs following hospital discharge and provide tailored support to aid their recovery, promoting independence wherever possible over time.

YOUR MAIN RESPONSIBILITIES:

  • Lead and manage a designated team of Reablement staff, including recruitment, workload allocation, supervision, performance monitoring, appraisals, and identifying training needs.
  • Ensure staff deliver outcomes-based support and goal plans for clients receiving Reablement or long-term care services in their own homes.
  • Conduct initial and follow-up visits with staff in clients’ homes to assess outcomes, perform risk assessments, and provide guidance to individuals, carers, families, and other professionals.
  • Collaborate with professionals and commissioning staff to align on assessed goals and outcomes for clients.
  • Maintain accurate client records and ensure all service documentation remains up to date.
