We are Surrey Heath Community Providers Limited, which is a federation of 7 GP practices across 10 sites, covering a population of over 97,000 patients across Surrey Heath. We began in 2016 and now employ around 200 members of staff across our clinical and corporate support teams. Our services include Surrey Heath Primary Care Network, PRIMACY workflow management and Upper Gordon Road Surgery.

  • In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately.
  • In the performance of the duties outlined in this Job Description, the post-holder may have access to confidential information relating to patients and their carers, Practice staff and other healthcare workers. They may also have access to information relating to the Practice as a business organisation. All such information from any source is to be regarded as strictly confidential.
  • Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorised persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Responsibilities

ROLE SUMMARY:

To provide a point of contact for patients. To facilitate effective communication between patients and the Primary Health Care Team and to promote a positive image of the practice to its patients and service users.

DUTIES AND RESPONSIBILITIES:

  • Opening the premises and checking heat and ventilation.
  • Preparing forms etc. for clinics as necessary.
  • Sorting of prescriptions for collection.
  • Booking appointments both by phone and at the front desk, receiving calls and making outgoing calls / sending messages as required to facilitate appointments.
  • Using various computer software systems including the practice clinical system.
  • Accessing computerised patient medical records for the purpose of carrying out your duties.
  • Providing sign-posting to patients to appropriate support, in line with practice policies, procedures and guidance.
  • Ensuring patients’ confidentiality at all time in line with practice protocols and policies.
  • Receiving and directing patients on arrival in a courteous and efficient manner.
  • Answering general enquiries, explaining surgery procedures, making new and follow up appointments and receiving requests for repeat prescriptions.
  • Receive and respond to requests from patients or practice team in relation to accessing the practice, including the use of the practice system(s) for online consultation and clinical and administration queries, ensuring this is supported and used effectively as the main route of access for patients, regardless of the mechanism of access (i.e. walk-in/in-person, telephone, online) and across the full operational hours of the practice.
  • Undertaking relevant training and acting as a chaperone as required.
  • Undertaking training and acting as building fire marshal/warden, ensuring evacuation lists are current and that the visitors log is used appropriately.
  • Undertaking administration to support the new patient registration process: Issuing practice information, obtaining completed questionnaires, checking completed registration forms and issuing New Patient medical check appointments.
  • Entering data onto the computer when required.
  • Preparing repeat prescriptions on the computer when required, in preparation for doctors to sign.
  • Filing as required to patient notes.
  • Extracting patients notes when required by a doctor.
  • Printing and managing patient records for removal from the practice list.
  • Manning telephones: making and receiving calls, transferring calls accurately, booking patients in for appointments.
  • Receiving and logging requests for home visits, telephone calls etc.
  • Ensuring that there is an adequate supply of stationery available in the consulting rooms and reception area.
  • Logging any messages received for nurses, health visitors, midwives and counsellors in the appropriate books.
  • Tidying up waiting rooms & public areas including doctors consulting rooms at Upper Gordon Road Surgery.
  • Franking post and taking it to the post-box daily also posting parcels at Post Office as and when necessary.
  • Photocopying documents and shredding documents no longer required.
  • Locking up & securing the premises at night and carrying out the various checks this procedure entails.
  • Undertaking duties as may be determined from time to time commensurate with the range of activities described above.

13. CARRY OUT ANY OTHER DUTIES AS REASONABLY REQUIRED BY THE TEAM LEAD, PCN/ FEDERATION, CLINICAL DIRECTOR OR PRACTICE MANAGER.
