Position Summary The Recreation Specialist carries out the program of recreational activities of the Home by direct participation, supervision, and instruction of the Members participating in the program; assists in the over-all planning of the Recreation program.  Support Culture of Excellence through active role with assigned neighborhood initiatives. Essential Job Functions: * Prepares for, sets-up and conducts scheduled activity groups as assigned by supervisor.  Orders and/or purchases supplies with approval from supervisor.    * Responsible for all required medical record documentation on assigned Member caseload; includes assessments for MDS completion, initial, transfer, re-admission, change of condition, behavior and quarterly assessments. Utilizes in-house computer network programs to complete medical record documentation. * Plans and coordinates services on special events and/or projects as assigned by supervisor. * Plans for 1-1 interventions and/or visitations with those Members not actively participating in group programs. * Prepares for and attends Patient Care Conferences as assigned.  Provides input for interdisciplinary team and is responsible for CAAs when triggered. * Responsible for supervision of volunteers assigned to assist with programs. * Keeps an attendance record of all Members visited on a 1-1 basis or in attendance at groups. * Covers weekend programs as scheduled and may assist in evening programs when needed. * May be assigned to take Members on outings and is responsible to meet their needs and to maintain their safety. * Recreation Specialists assigned to dementia units prepare monthly schedule of activities with input from unit staff. * Helps transport Members to recreation programs when needed. * Makes posters, sign-up sheets and/or visits to keep Members informed of programs offered and to encourage their participation. * Participates in service education and safety programs. May attend selected meetings and workshops to improve job skills. * Performs other duties as required by supervisor or by Administration as directed. Expectations of the Position: * Dependable, report to work on-time for every schedule shift (ready to work at beginning - start of shift) and work full schedule shifts unless physically unable or pre-approved Paid Time Off (PTO) which includes personal and vacation. * Willing and able to work overtime and weekends as required by the position and/or approved by management. * Have reliable transportation to and from work. * Must be clean and neat in appearance and have a “friendly” service-oriented demeanor and supports Culture of Excellence initiatives by interactions with co-workers and/or Members. * Abide by work-related / job procedures and organization policies; (refer to the Employee Handbook). Qualifications of the Position: * Education:  Bachelors or A.A.S. degree in Recreation or other health related field (Bachelor’s preferred).  * Experience: At least six months in recreation therapy.  Long term care experience preferred. * Skills:  Ability to lead groups and work comfortably with memory impaired individuals.  Ability to communicate well and to work with others.  Ability to work independently.  Must have excellent assessment and writing skills. * Licenses / Certifications:  Must have valid NYS driver’s license.
