Responsibilities

Anishinaabe Miikan, Red Road to Recovery Program is an Indigenous Guided Mental Health and Addictions Program. Partnering with local indigenous organizations, we are able to provide a wider range of supports for those in need. Red Road to Recovery looks at the physical, psychological, and spiritual aspects of addiction – while addressing the individual needs of participants.
As a part of the interprofessional primary care team, the Red Road to Recovery Program, Case Manager’s role is to support the development of a comprehensive supportive treatment plan, coordinate physical, emotional, mental, spiritual, and social health services, and to make appropriate referrals and linkages to community services and resources. The Red Road to Recovery Program, Case Manager provides services in a supportive, confidential, and culturally safe manner to help clients work towards their goals and access the resources they need to fully participate in health care services. The Red Road to Recovery Program, Case Manager is responsible for case management specific screening, assessing, planning, connecting, and transitioning of care. The care provided by the Red Road to Recovery Program, Case Manager is time limited, and goal focused. The Red to Recovery Road Program, Case Manager will participate in rounds, team meetings, care conferences, and client appointments as necessary to support the fulfilment of their case management plan. Although this role will work closely with therapists and social workers, the Red Road to Recovery Program, Case Manager does not provide therapy or counselling to the clients they support. The Red Road to Recovery Program, Case Manager will receive care guidance from Addictions and CWS Team members in the services they provide. Clients will be connected through the CWS program, Addictions related programs as well as other areas of Mamaway Wiidokdaadwin as assigned.
