POSITION SUMMARY: The Reentry Case Coach will work with incarcerated individuals to assess their needs, develop case plans, and monitor progress as they prepare for successful reentry into Dauphin County within six months. Serving participants in both state correctional institutions and local prisons, the Case Coach provides direct assistance to justice-involved individuals, ensuring they have the resources and support to overcome barriers to reentry. This role requires collaboration with partner agencies to facilitate services related to employment, training, education, and other supportive needs.

  • Demonstrated capacity to handle multiple tasks, projects and meet deadlines.
  • Excellent public relations, community organizing, negotiation, mediation, conflict management and meeting facilitation skills.
  • Knowledge of the Dauphin County Justice System and/or community resources with demonstrated experience working with justice involved young adults.
  • Demonstrated ability to maintain confidentiality with sensitive information.
  • Demonstrate ability to inspire community participation and build coalitions.
  • Computer literacy in word processing, email, internet, and spreadsheets.
  • Must have strong administrative and organizational skills
  • Demonstrate excellent verbal and written communication skills.
  • Ability to develop strong relationships with diverse groups of citizens, who may have conflicting interests and opinions.

MINIMUM QUALIFICATIONS:

  • Bachelor’s Degree in Social Work (BSW), Psychology, Counseling, Criminology or a relate social/behavioral science field.
  • Master’s degree in social work (MSW) provides distinct advantage
  • 2-3 years’ experience in delivering services to individuals involved in the Criminal Justice System.,
  • 2-3 years’ experience in delivering case management services
  • Ability to pass criminal background, State, Local and Federal
  • Excellent verbal, written and interpersonal communication skills
  • Ability to work effectively with people of diverse educational and cultural backgrounds, from ex-offenders to public officials
  • Computer literacy, including proficiency with MS Office applications and database use.
  • Knowledge of criminal justice system and/or experience with ex-offenders is preferred.
  • Ability to travel within city and surrounding communities. Select national travel also possible.

    Pay: $19.00 - $21.89 per hour
    Expected hours: 37.5 per week

Responsibilities
  • Service a caseload of 25-30 active participants, all of which will be justice involved young adults.
  • Manage a caseload of 25-30 active participants, all of which will be incarcerated at the beginning of the Program.
  • Conduct individual case management sessions with clients on a regular schedule both in a prisons and office
  • Perform initial screenings to determine the level of service and support needed.
  • Develop individualized case plans addressing barriers to successful reentry.
  • Maintain accurate documentation of all client interactions, including contact details, outcomes, and action plans.
  • Provide or coordinate holistic services to address participants’ needs.
  • Regularly update the Program Manager on client progress and program status.
  • Attend program-related meetings, trainings, and conferences.
  • Work closely with program funders to collect data, write reports, and conduct follow-up activities.
