Schedule: Tuesday/Thursday, 8:00 am - 5:00 pm ECHO Community Healthcare is a Federally Qualified Healthcare Center that provides quality healthcare to all members of our community, including the insured, uninsured, under-insured and homeless. We currently employ over 150 dedicated team members who truly embrace the mission of serving and making a difference in the lives of others in a positive, supportive and collaborative environment. We serve a population of approximately 15,000 and provide various clinical and community outreach services at six full time clinics. NO EVENINGS, NO WEEKENDS-13 PAID HOLIDAYS! BENEFITS DAY 1! Job Summary The Referral Agent appropriately and efficiently gathers needed information from external specialty services for established patients and internal referrals. The Referral Agent works closely with Providers and Site Manager to evaluate patient satisfaction and outcomes of care and to provide coordination of services for patients within an integrated health care system. SKILLS: Demonstrates effective written and verbal communication skills Concern for client’s dignity EMR (EPIC) experience preferred Healthcare setting: Clinic Schedule: Monday to Friday Day shift Please, no phone calls.
