Job Details Job Location: SFC - Spring - Spring, TX 77388 Position Type: Full Time Education Level: High School Diploma/GED Job Category: Other Positions US Heart and Vascular is in need of a Full-time Intake and Referral Coordinator to join our team at Samual Family Cardiology in Spring, TX Responsibilities: Receive, review, and process cardiovascular patient referrals Verify insurance and benefits Maintain accurate and timely referral logs and dashboards for tracking. Flag high-risk referrals Follow up with referring physicians regarding appointments or inability to reach patients Document all referral activities within the EMR/EHR accurately and promptly. Support reporting on referral volume. Work closely with Business Development team, Practice Manager, and frontline staff to ensure excellent patient service and maintain referral relationships Participate in team huddles to review pending referrals Provide feedback from patients and referring providers to improve workflows and communication. Requirements: Communication Skills: Strong verbal and written skills with ability to explain care program details clearly to patients and providers. Organizational Skills: Ability to manage high volumes of referrals, track multiple patients, and prioritize follow-up in a fast-paced environment. Technical Proficiency: Skilled in EHR/EMR platforms, Microsoft Office/Teams, and basic data entry/reporting.
