We are looking for: Someone who can provide services for new and returning patients, schedule appointments, and obtain referrals when it is necessary in order to coordinate patient care. Responsibilities: Coordinates care between physician and patient and completes incoming/outgoing requests for referrals. Obtains additional information from referring physician such as labs, imaging, and progress notes. Confirms authorization and referral information and verifies if additional information is necessary Establishes why the patient is seeing the physician and what is the associated diagnosis code. Calls patients to schedule appointments if an incoming referral has been received or if there is an outgoing referral.
