Homelessness is the largest social and public health crisis in California. Illumination Health + Home is a growing non-profit organization dedicated towards disrupting the cycle of homelessness by providing targeted, interdisciplinary services in our recuperative care centers, emergency shelters, housing services and children’s and family programs. IHH currently has 13+ facilities with 22+ micro-communities scattered across Orange County, Los Angeles County and the Inland Empire.

JOB DESCRIPTION

The Referral Coordinator plays a critical role in the intake and coordination process for homeless clients referred to from hospitals and other medical or social service agencies. This position ensures that all incoming referrals meet admission criteria for the Recuperative Care program, coordinates with healthcare providers for client discharges, and maintains compliance with all contract requirements. The Referral Coordinator serves as a liaison between Illumination Health + Home and referring entities across multiple counties to facilitate seamless transitions into care. Additionally, the role includes administrative responsibilities to support accurate tracking, reporting, and billing.
The pay range for this role $20-$22 per hour.
The Schedule for this role will be Monday/Friday, 2:00pm-7:00pm, Wednesday/Thursday, 12:30pm-9:00pm.

PREFERRED EXPERIENCE/ MINIMUM QUALIFICATIONS

Required:

  • High School Diploma or GED
  • Minimum of one (1) year of experience in case coordination, patient navigation, medical administrative support, or a related field
  • Strong organizational skills with attention to detail
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel)
  • Excellent written and verbal communication skills
  • Ability to manage time effectively in a fast-paced, multi-agency environment

Preferred:

  • Associate or bachelor’s degree in public health, Healthcare Administration, Human Services, or a related field
  • Experience working with homeless or medically vulnerable populations
  • Familiarity with Managed Care Organizations (MCOs), IPAs, or hospital discharge procedures
  • Experience using electronic health record (EHR) systems or referral tracking tools
  • Bilingual in English and Spanish
