REFERRAL COORDINATOR
Lakeland Primary Care Network is inviting applications for the Referral Coordinator (RC) position (0.5 FTE, 37.5 hours bi-weekly) on one year temporary contract at PCN Hub, St.Paul.
The Referral Coordinator (RC) position will serve patients of the clinic by greeting, welcoming, directing and announcing them appropriately. The primary goal of a Referral Coordinator is to enhance the patient’s care experience, improve clinical outcomes and quality of life.
EXPERIENCE & SKILLS:
· Ability to work within tight deadlines and prioritization of work to achieve them.
· Strong analytical, critical thinking, troubleshooting, problem-solving skills, and a high degree of accuracy and attention to detail.
· Superior written and verbal communication skills.
· Able to succeed and adapt to a fast-paced environment.
· Able to work independently under minimal supervision.
· Advanced technical proficiency in Microsoft Office Suite (e.g. Excel, Word, Visio, PowerPoint, Outlook).
· Effective Communication and Interpersonal Skills.
· Ability to work cooperatively in a multi-disciplinary team environment.
SCOPE OF THE ROLE:
Referral coordinators typically work in healthcare settings where they assist patients by coordinating referral appointments and working with insurance companies and care providers to share essential patient information. They may also perform general administrative or clerical tasks such as answering phone calls and emails, maintaining patient records, and verifying insurance information before appointments. Referral coordinators frequently interact with patients and doctors, so they need some experience with customer service tasks and the ability to maintain patient records and confidentiality, manage multiple schedules, and ensure that patient information is complete and up to date.
