Description:
REFERRAL COORDINATOR
Lakeland Primary Care Network is inviting applications for the Referral Coordinator (RC) position (0.5 FTE, 37.5 hours bi-weekly) on one year temporary contract at PCN Hub, St.Paul.
The Referral Coordinator (RC) position will serve patients of the clinic by greeting, welcoming, directing and announcing them appropriately. The primary goal of a Referral Coordinator is to enhance the patient’s care experience, improve clinical outcomes and quality of life.

Responsibilities:
SCOPE OF THE ROLE:
Referral coordinators typically work in healthcare settings where they assist patients by coordinating referral appointments and working with insurance companies and care providers to share essential patient information. They may also perform general administrative or clerical tasks such as answering phone calls and emails, maintaining patient records, and verifying insurance information before appointments. Referral coordinators frequently interact with patients and doctors, so they need some experience with customer service tasks and the ability to maintain patient records and confidentiality, manage multiple schedules, and ensure that patient information is complete and up to date.
DUTIES & RESPONSIBILITIES:
- Maintaining confidential electronic medical records, e-mail and paper filing systems.
- Complete referrals to specialists using designated software and other computer applications.
- Ensure assigned physicians’ complete patient specific forms required for specialist referrals and any required investigations have been completed and included with referrals.
- Participating on special projects as required.
- Develop linkages with various community health resources to deal with patients requiring treatment and referral.
- Continually updating one’s own knowledge of community services/information that is available to physicians and patients.
- Scheduling patients with visiting specialists.
- Provide all required reports to the Executive Director from time to time.
- Ensures adherence to Lakeland PCN policies, HIA and PIAs in preparation of data sharing agreements, data transfer, and record retention with respect to evaluation.
- Prepare treatment areas in between patients.
- Maintain the cleanliness of the clinic including daily/weekly cleaning duties and some light housekeeping.
- Other duties as assigned.
