Description
The Referral Coordinator supports patient care by coordinating referrals to external specialists, providers, and diagnostic services. This role verifies insurance coverage, schedules appointments, and serves as a liaison between patients, providers, and insurance companies to ensure timely access to care. The ideal candidate is organized, detail-oriented, and committed to delivering exceptional patient service in a fast-paced healthcare environment. Duties and Responsibilities   * Coordinate and process referrals * Coordinate and process hospital discharges * Submit and track authorizations * Interface with corporate management, administrative personnel, medical, dental and behavioral staff * Identifies the health needs of the patient and the expectations of the family. * Makes an assessment of the resources and support network at the patient’s disposal. * Assists in development, implementation, and revision of individual treatment plans; assures that services provided are specified in the Treatment Plan; and monitors progress toward treatment goals. * Maintains an active caseload, providing interventions as needed and within area of expertise and limits of credentials; assures procurement of additional services as needed. * Reviews and facilitates, if appropriate, mental health referrals from other sources such as primary care, school health, etc. Offers alternative recommendations where necessary. * Maintains patient and program records in accordance with applicable standards and regulations, grant requirements, etc. * Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature. * Participates in continuing education activities, remaining knowledgeable in area(s) of expertise. * Attend meetings, as appropriate, and meet regularly with supervisor to exchange pertinent information and receive supervision. * Participates in all safety programs, which may include assignment to an emergency response team. * Other responsibilities as assigned to create a smoothly run efficient Clinic.   Education   Bachelors Degree or equivalent work experience  Qualifications  Work requires a minimum of 6 months experience in responsible clinical/medical operations or equivalent referral coordination experience.   Excellent communication skills and proper phone etiquette required.  Must be computer literate.  Experience with a variety of business machines.  Demonstrated ability to multi-task in a fast paced medical center environment.      PanCare provides a comprehensive benefits package to include medical, dental and vision insurance.
