Our supportive culture is built on respect, friendliness, and a shared commitment to exceptional patient care.   What You'll Do The Patient Referral Coordinator oversees and coordinates patient referrals to ensure timely, accurate, and seamless care and follow-up. This role works collaboratively with providers, patients, and external partners to facilitate the referral process, maintain precise documentation, and promote continuity of care across the patient journey. * Coordinate and track patient referrals from initiation through completion, ensuring timely and seamless care * Verify patient demographic, insurance, and clinical information, and obtain prior authorization as needed * Communicate with patients to explain referral details, next steps, and assist with scheduling appointments and follow ups * Maintain accurate documentation in the electronic medical record, including referral status and related communications * Collaborate with providers, clinical teams, and external partners to support effective care coordination and continuity of care * Performs a variety of tasks and other duties as assigned   Who We're Looking For We’re seeking a detail-oriented, adaptable professional with strong communication skills who thrives in a fast-paced environment.
