Overview The Referral & Diagnostic Coordinator supports the mission of People's Community Clinic by assisting nursing and medical staff with coordination of patient care with outside entities including medical specialist and diagnostic facilities to ensure efficiency and continuity of care. All PCC employees are expected to identify opportunities to improve workflow, work processes and patient care and to work cooperatively to implement those improvements. This position works as a member of a care team to coordinate patient care and support patients and families in self-management, self-efficacy and behavior change. Care team members are expected to assess and manage the health needs of the patient population and to communicate effectively with all patients, especially vulnerable populations. Responsibilities Under the supervision of Referral and Diagnostics Supervisor, this individual’s primary responsibilities include: Coordinate patient care by verifying patient insurance and ensuring authorizations and referrals are obtained for appropriate diagnostic exams and specialist visits. Maintain effective communication with patients and relevant clinical staff regarding upcoming diagnostic or specialist appointments and follow up to ensure patients are prepared, appointments are kept, and results are returned to PCC. Maintain electronic health records ensuring referrals are closed and diagnostic results are entered daily, reports are uploaded to appropriate provider PAQ, and alert providers regarding any STAT results. Manage task box and use reporting systems to track follow-ups and successful outcomes. Adult and Women’s health - outreach and follow up as part of preventative care screenings including Mammography Identify opportunities to improve workflow, work processes and patient care, and to work cooperatively to implement those improvements. Adhere to HIPPA Guidelines. Qualifications Education: High School or GED, Certified Medical Assistant preferred. Experience: One year working in a medical office, customer service and general office experience. Knowledge, Skills, and Abilities: Bilingual in English and Spanish required. Excellent customer service skills. Sharp attention to detail. Strong ability to multi-task. Good verbal and written communication skills. Strong ability to manage time and prioritize. Adequate computer skills, including data entry, and familiarity with electronic records. Strong Ability to research and make sound logical decisions.
