Job Description Position: Reflections Coordinator-LPN Employment Type: Full-Time Location: Brandywine Living, 132 Warwick Rd, Haddonfield, NJ Pay Range: $38 - $39 per hour Final compensation will be based on experience and qualifications. Job Summary: Plan, organize and manage with oversight of the RN Wellness Director all clinical operational aspects of the Reflections program by ensuring the highest degree of dementia care is provided to the residents through the implementation of all Monarch/Brandywine standards, dementia training, and overall clinical operations for memory care. In addition, ensuring all applicable federal and state guidelines and Brandywine policies and procedures are met. Key Responsibilities: Coordinate recruitment and hiring process for the Reflections Care Management team members Complete staff performance appraisals/disciplinary action Facilitates general orientation for all staff on Reflections Program Responsible for environmental rounds to check for safety of exit doors, resident room safety and unit safety. Cooperate with other resident services to ensure that the resident's total regimen of care is maintained. Report to the RN to assess and document resident and family response to interventions. Prepare and administer medications as ordered by the physician. Qualifications Active LPN license CPR certified Experience in Long Term Care or Assisted Living is preferred. Strong clinical, organizational, and communication skills. Graduation from an accredited School of Nursing, with current LPN license At least 2 years’ experience with long-term care, assisted living, home health or hospital setting preferred Experience working with residents with Alzheimer’s or other related dementias Ability to handle multiple tasks Knowledge of federal and state regulations, and of nursing practices, techniques and methods applied to health and wellness resident While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell. An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
