Description
Regional Care Coordinator Full-Time The Regional Care Coordinator is responsible for overseeing, coordinating, and enhancing patient care services across multiple sites within a designated region. This position works collaboratively with local care teams, administrative leadership, and external providers to ensure consistent, high-quality, patient-centered care. The role includes monitoring compliance with care protocols, supporting case management efforts, and driving performance improvement initiatives. Your Role: Serve as a liaison between regional sites, physicians and other providers, payer partners and corporate care management to ensure alignment of care standards and practices. Coordinate care transitions between payers, hospitals, specialists, and community services to ensure continuity and reduce ER visits and hospital readmissions. Monitor and evaluate regional performance metrics, including utilization, quality indicators, and patient outcomes. Maintain knowledge of payer requirements, community resources, and current healthcare regulations impacting care coordination. Education: Must have completed minimum of an associate's degree as a Physical Therapy Assistant, or related field; Bachelor's degree preferred. Experience: Minimum 3–5 years in physical therapy or adjacent field; experience in a multi-site or regional role preferred. Skills: Strong organizational and time-management skills, experience with scheduling via EMR preferred. Flexibility to adapt to changes in schedules or job priorities Excellent communication and interpersonal skills, written and verbal. Excellent customer service skills.
