What makes this role different Lead clinicians — not just operations You’ll mentor and develop Primary Care Providers across multiple clinics, supporting them in delivering relationship-based, whole-person care. You’re not managing from a distance — you’re in it with them. Stay connected to patients This is predominantly a leadership role with a clinical care component, where you will maintain a small patient panel to stay closely connected to the patient experience. You’ll partner with senior leadership to open new clinics, develop teams, and scale a care model designed specifically for older adults with complex needs. What you’ll do · Carry a small patient panel while providing clinical leadership across multiple care centers in the Boston market · Lead day-to-day clinical operations, including patient flow, care delivery, and team workflows · Mentor, develop, and performance-manage Primary Care Providers and clinical staff through coaching, case discussions, and real-time problem solving · Drive clinical quality outcomes while improving patient experience and implementing continuous quality improvement initiatives · Lead clinical huddles and foster strong interdisciplinary collaboration across care teams · Advance utilization management strategies to reduce avoidable ER visits and hospital admissions · Partner with leadership to execute growth strategy, including new clinic openings and expansion efforts · Support hiring, onboarding, and training of clinical teams across the region · Collaborate with operations, growth, and community teams to improve patient access, engagement, and outcomes · Use data, supported by our Gather Connect technology to continuously improve care delivery and performance Requirements · Board Certified in Internal Medicine, Family Medicine, or Geriatrics (MD/DO) · Active Massachusetts medical license, or ability to obtain prior to start · 7+ years of clinical experience in adult, family, or geriatric care · 3+ years of clinical leadership experience · Experience in value-based care models · Strong clinical judgment and proficiency in evidence-based primary care · Ability to lead, coach, and inspire teams in a collaborative, low-hierarchy environment · Comfort working with the latest care delivery technology platforms · Strong ethics and ability to navigate complex, sensitive situations Preferred: · Experience with HEDIS, STARS, or similar quality frameworks · Background in utilization management · Experience working with underserved or socially complex populations
