Description A Long-Term Care (LTC) Dietitian is a Registered Dietitian (RD) responsible for assessing, planning, and managing the nutritional health of residents in nursing facilities or skilled nursing homes. They work within an interdisciplinary team to create personalized care plans, manage chronic conditions, monitor weight/lab values, and ensure compliance with CMS and state regulations. Key Responsibilities Nutritional Assessment & Care Planning: Conducting comprehensive nutritional assessments upon admission, quarterly, and upon significant change in condition to develop individualized, person-centered care plans. Clinical Nutrition Care: Monitoring and addressing clinical issues like unplanned weight loss, pressure ulcers, dysphagia, and dehydration. Documentation & Compliance: Maintaining accurate, timely medical record documentation to ensure compliance with federal (CMS) and state regulations. Interdisciplinary Collaboration: Actively participating in care conferences, weekly wound rounds, and quality improvement (QAPI) meetings. Menu & Dining Service Management: Auditing food service operations, including kitchen sanitation and menu planning to ensure high-quality, appealing meals. Education: Providing nutritional education to residents, families, and staff regarding therapeutic diets and nutrition interventions. Requirements Required Qualifications Active Registered Dietitian (RD/RDN) credentials. State licensure (if required by the state). Strong communication and interpersonal skills. Experience in long-term care or geriatric nutrition is preferred
