Overview About the Role The primary purpose of the Registered Dietitian position is to assess and intervene with Adult (18-64 years) and Geriatric (65-95+ years) clients, for nutritional risk and education requirements. Responsibilities What You’ll Do Performs nutrition assessments in the time frame consistent with the N.J. State Health Code and/or policy and procedure for designated subunits, and triages clients into appropriate level of care to determine time frame for follow-up intervention and intensity of nutritional care required. Documents all nutritional care provided to clients in the medical chart using the S.O.A.P format or as outlined in the policy and procedure for designated subunits. Evaluates and monitors client’s progress consistent with policy and procedure and implements appropriate nutritional care collaboratively with primary care physician or designate. Attends and actively participates in interdisciplinary conferences anddischarge planning Demonstrates clinical competency in all areas of clinical nutrition by maintaining 15 CEU’s per year, participation in peer review and cross training educational activities. Develops and implements clinical research projects collaboratively with clinical nutritionstaff, ADCN and/or medical/nursing staff. Counsels and educates patients and significant others on nutritional requirements and therapy, inclusive of, but not limited to Diabetes, Heart Failure, and Obesity. Develops instructional materials with posttests to support client learning and ability toprovide self-care. Maintains accurate nutritional records and communicates appropriate nutritional care to the primary care physician or designate; and to the ADCPS during clinical rounds and peer reviews Qualifications What You’ll Bring Required: Bachelor of Science Degree in Dietetics or Nutrition or a related field plus registration by the Academy of Nutrition and Dietetics. Preferred One (1) year work experience preferred as a Registered Dietitian in healthcare, including hospitals, nursing homes, long-term care, clinics.
