Apply today to learn more about our vibrant community!    We are seeking a Part Time/Temporary Registered Dietician to cover a leave of absence to start in Mid-June. The Registered Dietitian Nutritionist (RDN) provides nutrition consultation and education for patients and their families who are screened at nutrition risk, identified with nutrition risk, and/or referred by physicians or other healthcare providers for nutrition-related problems, chronic disease management, management of nutrition support, to meet regulatory requirements, or for prevention and wellness. The RDN is an integral part of the multidisciplinary team who serves as the patient’s nutrition advocate.    Requirements: Registered Dietitian   Preferred Experience: Long Term Care experience and experience with Point Click Care    Description:   Complete LTC assessments, assess all weight changes, complete MDs’s, assist with Subacute admissions as needed, complete consults at both the Meadows and Mayflower House. Attend morning meeting daily and IDT meetings on Wednesday and Thursday afternoons.
