JOB OVERVIEW

We are seeking a dedicated and knowledgeable part time staff Dietitian to join our multidisciplinary healthcare team. The ideal candidate will be responsible for assessing residents’ nutritional needs, developing personalized meal plans, managing changes in diet and swallowing risks based on changes in Dx and providing guidance on dietary choices to promote overall health and to improve nutrition related medical conditions. This role requires strong communication skills and the ability to work collaboratively with other healthcare professionals to ensure comprehensive resident care. The ability to communicate in Greek is an asset.

REQUIREMENTS

  • Registered Dietitian, with the College of Dietitians of Ontario
  • Proficiency in PCC electronic medical records systems for documentation purposes.
  • Bilingual skills are a plus, enhancing communication with diverse resident populations.
  • Strong computer operation skills for data entry and research purposes.
  • Excellent customer service skills to foster positive relationships with residents and their families.

Responsibilities
  • Conduct thorough assessments of residents’ dietary habits and nutritional needs based on the Fixing Long Term Care Act.
  • Develop individualized nutrition care plans based on resident assessments.
  • Provide education on nutrition, healthy eating habits, and lifestyle changes.
  • Monitor and evaluate residents’ progress towards their nutritional goals.
  • Document resident interactions and care plans in electronic medical records accurately.
  • Collaborate with physicians, nurses, and other healthcare staff to coordinate resident care.
  • Offer support and resources for residents with specific dietary restrictions or health conditions.
  • Stay updated on the latest research in nutrition and dietary practices.
  • Attends resident care conferences as required
