     Job Description   Responsible for nutritional care of patients. Plans, assesses, implements, and evaluates patient nutritional care plan. Assists in menu modifications and other nutritional related functions.  Job Responsibility   1.May mentor and train team members. 2. Responsible for accurately and  responsively performing varied clinical  tasks and problems of minimal  complexity 3.Interviews patient and/or family for nutritional history, obtains patient food preferences, assesses patients, and documents in patients’ medical record. 4. Conducts nutritional assessment, develops and implements patient specific care plans, and assists in recommending nutrition care plans to medical staff for review and consideration. 5.Recommends therapeutic diet modifications and/or supplemental feedings to physicians for patients, and ensures accuracy of diet order received by checking against physician’s order. 6. Provides nutritional guidance, education and counseling to patients and their families in accordance with the prescribed diet. Calculates nutrient content of tube feeding and dietary intakes as ordered by physicians. 7.Contributes to changes to patient’s menu, including patient food preferences, and the development of standardized recipes, as well as assists in overseeing food service workers in proper food handing and portion control. 8. Supports a clinical function to assist clinical team members and department or unit  management with carrying out daily  clinical operations 9. Prepares, presents, and conducts individual and group education classes, and refers clients to community services, as needed. Participates in and promotes Northwell wellness initiatives. 10.Develops and utilizes effective lines of communication regarding nutrition information within the program and community. 11.Provides in-services to culinary, food service and interdisciplinary teams regarding therapeutic diets and related topics. 12.Participates in Quality Assurance/Performance Improvement related tasks including data collection and executes tasks related to it, as directed. 13.Solves or escalates operational  problems and technical issues to senior  team members and management;  modifies existing tools or processes 14.Performs related duties as required. Job Qualification   Master's Degree or equivalent combination of education and related experience, required. Current Registered Dietitian Certification with the Commissions on Dietetic Registration, required. Certified Dietitian Nutritionist in New York State (CDN), as needed. Advanced/specialized certifications, as needed. 1-3 years of relevant experience, required. Oncology experience, preferred.
