HYBRID REMOTE POSITION JOIN A CARING, DEDICATED TEAM MAKING A DIFFERENCE IN YOUR COMMUNITY The Registered Dietitian/Nutritionist will provide and coordinate all phases of nutrition assessment, care planning, monitoring, education and counseling of patients. Develop a nutrition care plan based on an assessment of the patients’ nutritional needs, diet restrictions and current health plans 2. Consult with providers and health care personnel to determine as a team the nutritional needs and diet restrictions of patients in order to provide optimal patient care 3. Identify and provide individualized nutrition counseling recognizing the diversity among patients to meet age-specific needs and cultural backgrounds 4. Advise patients, families, and groups on basic recommendations for good nutrition, healthy eating habits, and proper food selection and preparation while guiding behavior modification to adopt these principles 5. Document in the electronic health record the Nutrition Care Process including the nutrition assessment, diagnosis, intervention, and monitoring of the patient 6. Review, update, develop and implement educational materials to meet the needs of patients and providers 7. Function as an integral team member of the Westside Family Healthcare clinical staff as well as collaborate with Westside’s Health Care Coordination team, Social Work team, and other functional areas in order to support patient needs in relation to their diet and nutrition goals 8. Develop knowledge of and good working relationships with external resources such as community organizations and education opportunities that can support the on-going health goals of the patient 9. Consistently implement evidence-based practices within scope of care 10. Stay current with national and state licensure education requirements and participate when required in professional education/certification programs 11. Participate in committees, advocacy and outreach efforts as requested Requirements Minimum Qualifications: 1. Bachelor’s degree in Food and Nutrition, Nutrition and Dietetics, Nutritional Sciences, Human Nutrition or comparable subject 2. Successful completion of an accredited dietetic internship 3. Current license as a Registered Dietitian in the State of Delaware 4. Ability to travel between worksites as needed Preferred Qualifications: 1. Master’s degree from an accredited institution with major in foods, nutrition, or dietetics 2. Fluency in written and spoken English and Spanish
