Clare Holland House provides Specialist Palliative Care Services throughout the ACT including Home-Based, Inpatient, and Outpatient settings along with outreach to Residential Aged Care Facilities (RACF) and North Canberra Hospital.
The Home-Based team provide Specialised Palliative care for patients in the community.
The team consists of Doctors, RNs, NP, and Allied Health (including Occupational Therapists, Physios, Social workers, and Spiritual Care supports).
The Home-Based Team Leader (TL) is responsible for the day-to-day service delivery by Specialist Palliative Care Nurses to our community patients and works Monday – Friday, 0700 – 1530, excluding public holidays.
You support the CNC, to make sure the team are safe, you ensure patients receive the care they need and the escalation when required.
