Your Opportunity:
The Diabetes RN provides expert nursing care, education, and consultative services to patients and families living with diabetes. Supporting patients both during clinic visits and through telephone/virtual interactions, the RN applies specialized knowledge and clinical expertise in diabetes education and management. This role plays a critical part in delivering comprehensive, patient-centered care within an ambulatory setting, working as a key member of an interdisciplinary team. The Diabetes RN is responsible for assessing, triaging, and coordinating care for patients in the diabetes program, utilizing clinical judgment, critical thinking, and the nursing process. Acting as a liaison between the interdisciplinary care team and external providers, the RN ensures continuity of care and supports patient self-management and education. In addition to diabetes-specific responsibilities, the RN may provide education to non-diabetic endocrinology patients and hospitalized patients requiring diabetes support.
Description:
As a Registered Psychiatric Nurse (RPN), you will provide leadership through a wide variety of nursing services to patients (clients, residents) for the purpose of promoting, maintaining or restoring health, mental health and preventing mental illness, injury and disability. You will teach, counsel and advocate on behalf of patients and take necessary steps to ensure their safety and well-being. You will provide safe, quality, patient- and family-centered care, while reflecting shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
