Your Opportunity:
This position is currently in the community health setting of Grande Prairie. This is the intake nurse position. You will primarily be working out of the Home Care office and responsible for managing and triaging referrals, leading morning meetings and ensuring that client and staff needs are met, as well as facilitate timely and effective communication between all members of interdisciplinary team. As a primary resource to families and staff, you will also need to have a solid knowledge of the community resources and agencies to help alleviate and manage client/family needs. You will assist the HCAs trouble-shoot issues with the medications assistance program (MAP) & work with the LPNs to identify risks to clients and possible interventions. You may need work directly with clients in designated living support facilities of 3/4/4D and in community clients’ homes, therefore prior case management experience is essential. You will bring to the position a strong nursing skillset in assessment & critical thinking, communication skills, positive attitude & willingness to work as part of a team as well as an ability to work independently. Because much of this position is done on the phone, you will manage high call volumes using professional and effective communication skills at all times while accessing and charting using a variety of computer-based platforms/databases.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.
