ABOUT THE ROLE

The Assertive Outreach Team is a tertiary level community mental health team providing care coordinated services to consumers diagnosed with a severe and enduring mental illness and who require an enhanced level of support to sustain living in the community.
The Assertive Outreach Team (AOT) based at Croydon Community Health Centre provides services to those living in the Canterbury and Croydon areas.
The Registered Nurse is responsible for the provision of patient centred; evidence informed nursing care that promotes identified health outcomes. The Registered Nurse will assess, plan, implement and evaluate nursing care in collaboration with individuals and the multidisciplinary healthcare team.
