Description

Essential Use their initiative and effectively manage their time and workload. Demonstrate competence with basic IT skills and Microsoft packages. Demonstrate a working knowledge of electronic medical records (e.g. EMIS) and be able to update records in line with organisational policy.
Awareness of the organisational strategic direction of the charity in regard to the provision of palliative care services. Organise workload and delegate tasks as appropriate. Demonstrate an understanding of frequently used medications and apply knowledge to ensure safe and effective practice. Demonstrate an awareness of palliative care emergencies.
Demonstrate involvement in service development. Foster good working relationships, communicating effectively with the MDT and service users internally and externally. Maintain strict confidentiality. Work with patients and those close to them with a kind, empathetic and person centred approach.
Demonstrate the ability to communicate sensitively and effectively with patients and their families about a range of complex issues including advance care planning. Demonstrate the ability to collaborate with patients to establish patient centred goals and develop a plan to achieve them. Support the delivery of hands on nursing care in conjunction with community colleagues. Deliver emotional and psychological support equivalent to level Key Responsibilities Clinical The RGN will be expected, under supervision, to deliver high standard of personalised care and identify needs of the patients through holistic assessment.
To use their experience and knowledge of palliative care to deliver appropriate symptom control, clinical and personal care, and medication administration. To work collaboratively with the Community Specialist Palliative Care Team (CSPCT), GPs and district nursing teams. To be skilled in communication To be responsible, support and provide clinical leadership to junior members of staff and Student Nurses. To be aware of cost effective and efficient use of resources.
To provide supportive care to patients, their families, and carers. To monitor patients on an ongoing basis, to evaluate outcomes and adjust care plans to ensure optimum co-ordination and delivery of care. Undertake bereavement visits as appropriate. To develop and maintain effective relationships with all members of the multi-disciplinary team to ensure collaborative working for the benefit of patient care.
To support the development and implementation of care pathways and patient information. To maintain accurate and up to date patient records (including electronic records). To provide a high standard of personalised and on-going care for bereaved families. To support and care for work colleagues whilst working in an emotionally stressful environment.
To ensure that the dignity, safety, and confidentiality of all patients is respected and that all patients receive the highest standards of care. Develop an autonomous approach within professional practice. Attend clinical meetings as required. To be aware of the principles of infection control in line with Charity Policy ensuring high standards of practice in self and others to promote a safe environment for patient care.
To be responsible for own professional actions guided by national, local and organisational policies, protocols and guidance. Professional To act as a clinical role model and to promote excellence in nursing practice. To maintain a valid NMC registration and act in accordance with the NMC Code of Professional Conduct. To assist in the provision of care in accordance with charity policies and procedures and with national clinical guidelines.
To be flexible in working pattern to meet the needs of the service. To keep up to date with professional and clinical developments. To demonstrate a commitment to research-based practice and clinical excellence. To undertake Personal Development Review (PDR) and to critically reflect on own performance through clinical supervision and mentoring.
Management To assist with the smooth running of the team. To undertake projects or duties as agreed with your Line Manager. To participate in quality improvement programmes. To participate in the development of clinical standards and clinical audit, working within the Clinical Governance framework at the Charity.
To contribute to the development of the services working alongside the registered and clinical manager. To record and monitor all incidents/complaints in involving staff, patients and visitors and lead on any investigations where required to do so by the Line Manager. To take an active role in promoting a healthy and safe working environment by ensuring compliance with all health and safety regulations and assisting in development and implementation of any policies to meet these standards. Training and Development To take responsibility for own continuing professional development and performance, including identifying own development needs.
To attend all statutory and mandatory training and ensure records of such training are kept up to date. To participate in the development of junior members of staff and take responsibility for their continuing professional development and progression. To ensure compliance with clinical competency assessments. To mentor, support and teach students on placement.
To orientate, teach and support new members of staff. To participate in regular reviews, audit projects and research activities under the supervision of the Line Manager. To participate in teaching and in-house training programmes. Organisational Support the registered and clinical manager in the delivery of strategic objectives.
Establish and maintain effective communications with internal/external agencies as appropriate. Attend and actively participate in external and internal meetings as appropriate. Ensure effective working relationships are fostered and maintained. Work in accordance with policies and procedures and any national guidelines.
