The Capacity Command Center (CCC) is a 24/7 multi-disciplinary, data-driven enterprise that carefully assesses health system operations to optimize the alignment of care teams and the coordination of patient activities. Our CCC staff leads the flow of patients and their continuum of care, from the time of admission to discharge, all from one place. Working in close collaboration with a diverse and high functioning team, the Transfer Center Nurse is a central contact for all incoming patient transfer requests to Cedars Sinai Medical Center. * You will coordinate all Transfer Center functions, including preadmission/admission screening, obtaining financial and medical clearance and working collaboratively with the sending facility representatives for safe and efficient transfers. * You will use experienced clinical skills to evaluate critical and non critical transfer requests. * You will also use your case management and utilization leadership skills to evaluate appropriateness of direct admits and incoming transfer as well expedite the transfer of our psychiatric patient population to inpatient Psychiatric Facilities.  
Responsibilities
The Transfer Center Nurse coordinates all incoming patient transfer requests and manages patient flow from admission to discharge. They evaluate transfer requests, ensure medical and financial clearance, and collaborate with multidisciplinary teams to optimize care alignment.
