Role: Registered Nurse Care Manager
Salary: £40,000 plus £3,000 Car Allowance
Location: Field Based with admin days in the office in Watford, WD17 1JJ

As such, and as we expand, we are offering the opportunity to join our team and are seeking an adult Nurse Care Manager. We are committed to providing the highest possible standards of care, and our Nurse Care Managers are responsible for the assessment and planning of care for adults in the community with complex health needs. They will provide leadership and management of the services provided by SHW, to the clients assigned to them. While also ensuring adherence to nursing standards and all related policies, procedures, and guidelines.
Key Responsibilities as a Nurse Care Manager:
- Uundertake assessments where necessary, in order to facilitate plans of care, and then to communicate with relevant colleagues.
- Work unsupervised and manage their time effectively to ensure all care plans, risk assessments, reviews, audits and associated tasks are completed according to the required standard.
- Analyse and interpret clinical information and medical evidence in order to provide a report in a professional and concise manner
- Work as part of a multidisciplinary team, building positive working relationships
- Apply professional skills and manage their own professional competence and accountability, in accordance with the NMC Code of Conduct
- Provide clinical support in an on-call diary
- Deliver programmes of education for Healthcare Support Workers for each package of care as appropriate.
- Collaborate with other Nurse Care Managers, Care Coordinators and the rest of the Operational Team to identify Healthcare Support Workers’ training and competencies needs.
- Conduct regular reviews of Healthcare Support Worker competencies and provide feedback and reports to the Registered Manager and the Operational Team.
What We’re Looking for in an Nurse Care Manager:
- Adhere to all Sugarman Health and Wellbeing policies and procedures.
- Adhere to the NMC Code of Conduct.
- Assess, plan, design, deliver and evaluate training and learning interventions.
- Keep meticulous accurate records of clinical training and competencies.
- Create and adapt training materials and learning delivery methods, and to deliver this in a range of settings including the service users’ homes. Assess the competency of Healthcare Support Workers and supervision sessions for the teams allocated to the clinical services
- Ensure all identified incidents are reported appropriately in line with Sugarman Health and Wellbeing policies and procedures.
- Understand the importance of audit in enhancing quality of care.
- Provide continued clinical input and support for staff in the daily delivery and support for individuals in their own home, ensuring that each person supported has their individual clinical care plans implemented and evaluated as required, in order to accommodate their changing needs.
