Superior Home Health Services are seeking a qualified, reliable and passionate Home and Community Registered Nurse Care Partner to join our team for Sunshine Coast region. You would be based at the North Lakes Head Office until Sunshine Coast Office opens and required to travel.

• Working within a person-centered, enabling environment that respects the rights and needs of each client.
• Manage case management for a variety of aged and disability clients.
• Manage Home Care Package funding administrative tasks;
• Assisting clients with personal hygiene, dressing, and domestic duties;
• Assisting clients with daily errands and transport where required;
• Providing support to access social and recreational activities;
• Assisting clients with medications;
• Observe, care and report any changes in the health and wellbeing of the client;
• Undertake skilled nursing tasks where required;
• Responding to emergency situations
