How you’ll make an impact in this role Lead the interdisciplinary care strategy from admission to discharge to ensure every patient experiences a seamless, safe, and well-coordinated transition home or to their next level of care. Advocate for patient needs and personal values by removing financial or logistical barriers, ensuring they receive the necessary resources and support for a successful recovery. Evaluate medical necessity and care levels continuously to ensure patients receive the right treatment at the right time while optimizing hospital and insurance resources. Collaborate with families, clinical teams, and insurance providers to build a unified plan that prioritizes high quality outcomes and prevents gaps in post discharge care. What minimum requirements you’ll need Licensure / Certification / Registration: Registered Nurse obtained prior to hire date or job transfer date required. Certified Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. ACM also accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire. What additional preferences we're seeking Minimum 3 year of recent acute inpatient nursing experience required. 1 year recent acute inpatient Case Management experience required.
