We Are Hiring Location: Pensacola, Florida Facility: Ascension Sacred Heart Pensacol Department: Case Management Schedule: Rotating | Full-Time What You Will Do Continuum Care Coordination: Coordinate the comprehensive, interdisciplinary plan of care from admission to discharge, integrating patient and family preferences to ensure seamless transitions across the care continuum. Clinical Monitoring & Advocacy: Monitor patient progress through continuous assessments and medical record reviews, actively advocating for necessary resources and the removal of barriers to care. Interdisciplinary Collaboration: Maintain ongoing dialogue with supervisors, physicians, and healthcare providers to ensure the effective implementation of the health plan and alignment of treatment goals. Utilization & Documentation Resource: Act as a key clinical resource for ensuring precise medical record documentation, verifying the appropriateness of services relative to diagnoses, and coordinating optimal post-discharge care options. What You Will Need Licensure / Certification / Registration: Licensed Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire dateor job transfer date required. Licensed as a Registered Nurse in Florida required. Credentialed from State Board ofNursing or current home state license for multi-state license recognition "Compact State" obtainedprior to hire date or job transfer date required. BLS Provider preferred. American Heart Association or American Red Cross accepted. Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time ofhire.
