REGISTERED NURSE CASE MANAGER Appalachian Agency for Senior Citizens/PACE * SUMMARY: Participates as a member of the Interdisciplinary Team (IDT) in the planning, implementation and evaluation of care plans that meet the objectives, standards and policies of the (Program for All Inclusive Care for the Elderly) PACE model of care. Demonstrates proficiency in administering nursing care in a professional and respectful manner with the goal of assisting frail elders to remain living in the community for as long as they can do so safely. Responsible for initial and ongoing assessment of health and nursing needs of PACE participants. Demonstrates positive interpersonal relations in dealing with fellow employee, managers, and other department employees to maximize positive employee relations and productivity. Exhibits appropriate communication skills necessary for ages served (adult/geriatric). Proactively involves the healthcare team in the care of the participant. Works collaboratively with interdisciplinary teams to meet the needs of the participant and family. Case manages and provides clinical direction to the interdisciplinary team, physician, and family. Implement/develop/document the participant plan of care. Assure continuity of quality participant care delivered with appropriate documentation. Perform on-call responsibilities and provide on-call service to participants/families as assigned. Hours / Schedule:  Full Time (40) hours per week Monday – Friday  MINIMUM QUALIFICATIONS: 1. Associates Nursing Degree required; Bachelor’s degree in nursing preferred. 2. Registered Nurse License  3. Minimum of one year’s experience working with a frail or elderly population. 4. Compliance with any federal or State requirements for direct patient care staff in their respective settings. 5. Valid driver’s license. 6. Current CPR Certification 7. Pass a criminal background check. 8. Medically cleared of communicable diseases and up to date with immunizations.  9. Responsibilities
