Direct Home Care - Registered Nurse Case Manager, Personal Care Aide Support Health Services of Northern New York is seeking a Registered Nurse Case Manager to support Personal Care Aide activities in St. Lawrence County. This position will be responsible for overseeing aide staff working in patient homes as well as performing admission and discharge services for personal care aide patients. An ideal candidate will have excellent communication skills and be comfortable traveling to and working one-on-one with patients and their families in their home. Additionally, the candidate will provide support and encouragement to our aide staff to help them succeed. This position will require travel to and from patient homes throughout St. Lawrence County. Additional travel may be required at times. This is a full-time position Monday – Friday with a Minimum salary of $66,000/yr.

Responsibilities
