Profession: Registered Nurse Specialty: Case Manager Location: Apple Valley, CA Job Type (Per Diem/Travel Contract): Travel/Contract Hourly Pay Rate: $62.90/hr Shift: Day Duration: 13 weeks Experience Required: 2 years of recent experience in the case management License Required: CA RN Certifications Required: BLS First-timers accepted: No Skills Required: Perform comprehensive patient assessments, including medical, psychosocial, and discharge planning needs. Develop and coordinate individualized discharge plans. Collaborate with physicians and the interdisciplinary care team to facilitate patient progression. Coordinate post-acute services including home health, skilled nursing facilities, inpatient rehabilitation, durable medical equipment (DME), and outpatient services. Conduct utilization review and ensure compliance with payer guidelines and regulatory requirements. Obtain prior authorizations and coordinate insurance approvals. Participate in multidisciplinary rounds and patient care conferences. Educate patients and families regarding discharge plans and available community resources. Maintain accurate documentation within the electronic medical record (Epic preferred). Ensure compliance with CMS, Joint Commission, and hospital quality standards. Current California Registered Nurse (RN) license in good standing. Current BLS certification through the American Heart Association. Minimum of 2 years of recent Acute Care Hospital Case Management and/or Discharge Planning experience. Acute Care Hospital experience required.
