The work is sometimes challenging and always rewarding by engaging with persons we serve and co-workers as we all learn, grow, and thrive.  Department: Hudson CCBHC  Pay Rate: $42-44 an hour Location: Hoboken, NJ Work hours: 32 hours per week (Wednesday/Thursday/Friday late hours until 8pm) The rest of the schedule is flexible. Position Overview: Specializing in medical and psychiatric care coordination for both children and adults. Will ensure proactive follow up for enrollees, medication reconciliation and care coordination within CCBHC services. Essential Job Functions Responsibilities: * Responsible for dispensing and documentation of medication, assisting the MD’s/APNs with medication and completing health assessments, care management, provision of health education for all ages. * Frequent monitoring and assessment of the mental and physical health status of consumers of all ages and provision of nursing care coordination * Nursing assessment and person-centered planning for integration of care to include assessment of stated concerns, BH and medical symptoms, response to medication and monitoring for medication side-effects. * Administer screenings for HIV and Hepatitis A-B-C, PPD, Metabolic conditions, vaccines, glucose monitoring, pulse ox monitoring, nebulizer treatment, EKGs, Carbon Monoxide testing, visual and auditory acuity screening and any SC or IM injections deemed necessary, and draw blood samples or collect urine samples for lab testing * Coordination and integration of services with DCO’s to include the FQHC, and other care and service providers * BH/Medical/SUD education, supportive counseling to consumers and significant others, often focusing medications, side-effects and physical health care, nutrition, exercise, integration of care and services * Education of the individual regarding his/her illness and the effects and side effects of prescribed medications, * Administers prescribed medications and injections, draws blood, and prepares laboratory specimens for delivery, if certified to do so * Responsible for accurate record-keeping of all medications administered in the Medication Record * Consults with DCO’s and other providers regarding the assessment, treatment and rehabilitation needs of the person served * Assess ADL’s and teach individuals needed skills * 24-hour on-call coverage on a rotating schedule as needed * As needed, will be trained on, and will provide culturally competent services which address the diverse needs of the population served and as identified on the needs assessment. * As needed, will be trained on, and will provide services in alignment with the clinical mental health guidelines promulgated by the Veteran’s Health Administration (VHA) and the Uniform Mental Health Service Handbook. Requirements: Education/Certifications: * Bachelor’s in Nursing preferred, RN license in NJ required. * CPR certification required. * Phlebotomy certification required. * Bilingual Spanish preferred.
