Position Overview: The Registered Nurse will provide compassionate person-centered care in a skilled setting to maintain the highest level of health and wellness in the residents’ daily lives. They are licensed to conduct their nursing practice independently. RNs are responsible for assessing residents’ condition, planning care, evaluating outcomes, providing delegations, and communicating any changes in health. Supervise and train the resident care teams and conduct assessments, develop care plans, and evaluate outcomes that ensure the health and well-being of each resident. Providing training, feedback, and mentorship to ensure staff are supported to do their jobs well. RNs are responsible for ensuring each resident receives the best possible care according to their health needs, personal preferences, and abilities. FLSA Status: Full or Part -Time Non-exempt, hourly Reports to: Senior Director of Programs Essential Job Functions Administer injections, medications, and provide wound care as needed Manage Therap reports/audits/etc. Perform resident assessments and participate in initial and ongoing service planning. Monitor status of resident’s health condition and initiate actions in response to changing needs. Prepare, coordinate, and implement direct care and health care services. Train, supervise, and oversee the daily care performed by nursing and resident care teams. Keep required records of resident admissions, transfers, and discharges. Coordinate daily shift services, programs, and activities to ensure residents care is maintained. Serve as liaison between facility, family, and medical personnel. Coordinate with other community health care professionals. Ensure completion of state-required documentation and forms Communicate clearly both verbally and in writing with staff, residents, and family members. Contribute to the development and implementation of community policies and procedures. Ability to work at multiple locations Delegate staff and meet OAR requirements for delegations Minimum Requirements: Approved RN license through the state of Oregon Stay up to date with OARs and licensing standards Certified in CPR and Oregon Intervention Systems (OIS) Strong interpersonal skills Ability to act swiftly in emergencies Knowledge of clinical skills Therap Knowledge Good communication skills Well-organized and apt in problem-solving Attention to detail Must pass Oregon criminal background screening. Must be able to remain awake and alert while on shift. Physical Requirements Must be able to lift up to 45 lbs., bend, and/or squat Must be able to walk up and down stairs Must be able to perform the job with or without a reasonable accommodation
